Parents of ROCC Youth students must complete this consent form giving permission for their child/children to attend any River Oaks Community Church Youth trips or activities during the 2026 summer and 2026/2027 School Year.

All Consent forms MUST BE COMPLETED BY A PARENT/GUARDIAN (referred to within as the Registration Contact) and not by the student attendee. Any consent form deemed by River Oaks to be completed by a student not of age WILL BE CANCELLED.
